Physician Orders for Life-Sustaining Treatment (POLST)

A POLST form  delineates the wishes of seriously-ill patients giving them more control over their end-of-life care. This could include medical treatment, what extraordinary measures such as a ventilator or feeding tube and CPR that  they would like to receive or opt out from.
