CHIA Membership or Colleague Application (New/Renew)
Signing up online is quick and easy. You can even renew! It’s a two-step process:

Fill out and submit the application  
Print your secure confirmation form


CHIA membership is contingent upon your current membership status with the American Health Information Management Association (AHIMA) (http://www.ahima.org/membership/).

Dues are calculated on an anniversary basis; your payment provides 12 full months of membership. To start or renew your membership please complete this application form, CHIA strives to maintain accurate, up-to-date contact information for all our members. This helps us provide you with timely mailings, important information, and valuable services.

If you don’t wish to complete the online membership application below and submit via the Internet, visit this alternative application: Active or Associate, Students. An alternate application has been posted here for Colleagues also.
