NOMINATION GUIDELINES
Requirements for award submission are as follows:
1) All supporting documentation must have relevance to specific criteria requirements. The volume of material submitted is not important. Documentation should support what the nominee accomplished and specifically why the nominee is deserving of an award.
2) The Nominator is responsible for ensuring that all required documentation is collected for the nominee. If any required material is missing, Executive Office staff shall immediately notify the nominator so they have the opportunity to provide the additional material. Missing material does not automatically remove a nominee from consideration, although incomplete submissions may adversely impact the committee’s ability to adequately and fully evaluate a nominee.
3) Nominees for all awards must be CHIA members (active, student, senior, honorary) or corporate partners of CHIA, with the exception of the Friendship award which may be presented to an organization or individual which does not meet this criteria.
4) Individuals who have previously won a particular award are not eligible to win that award again, even if it has been reformatted. They may, however, be nominated for a different award.
5) A nominee may not receive an award in a category for which he/she was not nominated.
6) Nominees not selected are not automatically considered in the following year for an award category unless the nomination is updated and re-submitted the following year.
7) All nomination materials become the property of CHIA. Materials cannot be returned.
8) Members of the CHIA Awards Committee and the CHIA President are ineligible for awards during their term of office. CHIA Board of Directors members, except the CHIA President, are eligible for awards during their term of service. CHIA staff members are eligible for awards during their term of employment.
9) Submit nominations on the CHIA Recognition Awards Nomination Form for Nominees and complete the form in its entirety.
10) The nominee’s resume or curriculum vitae is required.
11) Submit other written material, such as articles, project descriptions and the like, only if it supports the specific accomplishment(s) made, e.g., copy page of article or publication.
12) Provide two to four letters of support from others who can elaborate on:
o The nominee’s specific accomplishment(s)
o How the nominee has met the guidelines for the award for which they are being nominated, and
o Why the nominee is worthy of an award.
o Letters of support must be ’signed’ by the author and sent by fax, mail or via e-mail to authenticate and personalize the correspondence, preferably on letterhead.
GENERAL INFORMATION
Nominations with all required supporting documents must be in the CHIA Executive Office by the published deadline to be considered for that year.
CHIA staff shall verify if any nominees have previously received a CHIA Recognition Award; staff will indicate which award(s) the nominee has previously received, to ensure a previous award recipient isn’t awarded a different award for the same reasons cited previously.
Nominations not selected the previous year may be considered in the current year only if the nominee has been re-nominated for the current year. Letters of recommendation submitted in prior years can be accepted for the current year only if they have been updated (including new dates) and re-submitted for the current year by the authors of the letters.
All award recipients selected by the CHIA Recognition Awards Committee shall be notified of their selection prior to public announcement by the CHIA President or designee.
Awards will be presented annually at the CHIA State Convention & Exhibit. All award recipients are encouraged to be present to receive their award, but it is not mandatory. Recipients of awards may designate an individual to accept the award on their behalf.
Expenses for traveling to and attending the CHIA State Convention & Exhibit, including registration if desired, are the responsibility of individual award recipients.
